The goal of the management of Chambers Home Health Agency is to provide high-quality, efficient and effective health and health-related services in the least restrictive environment (primarily the residence) in order to promote dignity, mobility, independence, and well-being for the individuals in the agency’s liscensed service area. This mission shall include professional services and prioritized, quality patient care with a staff that is genuinely concerned in seeing that individual needs of the patients are met throughout the individual’s plan of care.
