Patient Services
Chambers Home Health provides high-quality, efficient
and effective health and health-related services in order to promote
dignity, mobility, independence and well-being for the patient.
Medicare, Medicaid and private insurance companies pay for many
of the costs incurred with home health nurses, home health aides
and therapy services. Patients can also request these services on
a private-pay basis. Medicaid reimburses for home health services
for patients who are enrolled in the State Medicaid Program.
Services offered by Chambers Home Health Agency include, but are
not limited to, the following:
Nursing
A registered nurse or LVN, under the close supervision
of the patient's physician, provides skilled nursing services such
as:
- Coordination and implementation of the physician's plan of treatment
- Diabetes management and instruction
- Sterile dressing changes
- Urinary incontinence with Foley catheter changes
- Instruction in cardiac rehabilitation
- IV therapy
- Medications - injections
- Care and instruction of colostomy, gastrostomy, ileostomy, and
tracheostomy
- Burn treatment
- Care of the terminally ill
- Laboratory services
- Factor VIII for hemophilia
Home Health Aide
Certified Home Health Aides (HHA) assist the professional nursing
staff by providing services of personal care such as:
- Bath
- Shampoo
- Shave
- Light cooking
- Linen change
- Simple household duties necessary to health care at home
Physical Therapy
Licensed Physical Therapists can provide rehabilitative care in
the home to restore the use of muscles
Occupational Therapy
The Occupational Therapists provide instructions in areas of self
care such as grooming, dressing, bathing and homemaking. The therapist
will also recommend methods of adapting the home environment to
make living at home easier.
Speech Therapy
The Speech Therapist provides assistance with communication disorders
that include eating/swallowing disorders, aphasia, and training
for the hearing impaired.
The Medicare Intermediary reimburses for home health care if the
following conditions are met:
- Home health services are ordered by the physician
- Patient is homebound due to injury of debilitating illness
- The patient requires periodic intermittent skilled nursing care
